SFA Application Process

All of the following forms and information must be submitted in the application portfolio for admission to the Surgical First Assisting program.
Incomplete application portfolios will not be considered for the program.
Applicants must be 18 years or older to sumbit your documents.

TO APPLY TO THE SFA PROGRAM PLEASE SUBMIT A PORTFOLIO WITH THE INFORMATION BELOW:

1. Cover letter that will include the following information:

  • Cover page with student contact information including: 
  • name, address, CSI student ID #
  • email address, 
  • phone number
  • CST certification number

3. Program skills checklist: Program Skills Checklist 

  • applicants should reach a 50% skill level prior to application 
  • Current CSI-ST students may submit their final scrub tally instead of the checklist.  It must be signed and dated by the ST instructor.

4. Current CPR/BLS certification

  • Must be American Heart Association Healthcare Provider CPR card
  • Must remain current for the entirety of the program

6. Background check and drug testing

  • The background check and drug testing is not due until students are ready to enter clinical facilities.  (to do clinical cases) however students should know that they MUST pass the background check and drug test to do clinical at our contracted facilities.

WHAT TO SUBMIT PRIOR TO ACCESS TO CLINICAL (clinical application documents) 
All of the following forms and information must be submitted PRIOR to registration for any clinical components of the Surgical First Assisting program.
Incomplete application portfolios will not be considered for the program.

  • Current health and immunization records:
    • History and physical exam
    • Eye exam
    • Current Immunizations
    • If you have been in the CSI ST program and have been a surgical services student continuously (without stepping out of the program for any reason) you may use the current health and immunization records submitted for the ST program.
  • Back ground check: CastleBranch.com .This is the only background check that will be accepted.

    • This information is required by the Fair Credit Reporting Act.
  • Malpractice Insurance:
    • All students will be required to obtain malpractice insurance that will remain current throughout the student's program of study in the Surgical First Assisting program. The insurance policy must be in the amount of $1,000,000 to $6,000,000 (one million to six million dollars) per occurrence. 
    • Coverage will be purchased through CM&F Group, Inc. at the following link for AST members:

      Please note at the bottom of the web page that practitioners that have become certified within the past year are eligible for a 50% credit. 
    • Please purchase the Surgeon Assistant policy under the self-employed heading. The listed price for this insurance is $200.00 prior to the 50% credit.
  • Mandatory random drug testing. 
    • You will be directed to obtain a random date drug test by your instructor. This is the only document that will not be included in the portfolio
