How to apply to CSI's Surgical Technology program
Welcome to the application process for The College of Southern Idaho’s SURGICAL TECHNOLOGY PROGRAM.
How do I apply?
Applications will be taken anytime however: They must be in by June 1st to be evaluated for the following fall semester start date.
- Apply to CSI (online) at https://mycsi.csi.edu/ics/Apply_to_CSI_Online.jnz
- Make application to the surgical tech program
- Take the compass test
- Meet with your assigned CSI advisor
- Meet with financial Aid Advisors if needed
- Contact Merry Olson at the CSI Career Center in the Taylor Building for instructions
- Create and submit a student admissions portfolio using the following information:
- A cover page with each of the following:
- Applicant's name
- Applicant's address
- Applicant's phone number
- Applicant's e-mail address
- Results of the his/her compass test
- Letter of reference from an employer
- Letter of reference from a professional
- Mandatory Background check
- The background check must be sent directly to the HSHSB building at CSI Attention Janet Milligan. The cost is $10.00 per student. The Idaho State Police will NOT accept personal checks. Students are responsible for making certain that the background check has been returned to the program manager’s before submitting their portfolio.
- Fingerprinting Services Must be included with the background check form:
Fingerprinting services are offered at most law enforcement agencies. Call the agency in your area to confirm the service, and to schedule a date and time to be fingerprinted. Remember to ask if there is a fee for the service. If you are coming to our location to be fingerprinted, you do not need to schedule an appointment. We are located at 700 S. Stratford Dr. in Meridian. We are open from 8:00 a.m. to 4:00 p.m., Monday through Friday, excluding holidays. Our fee is $5.00 for one fingerprint card and $10.00 for two or more cards, cash or check only. This does not include the background-processing fee.
- High School Transcripts and/or GED records
- College Transcripts (do not need to be an official transcript)
- C.N.A. certificate, EMT certificate, Medical Assistant Certificate if you have one (it is NOT mandatory). Please refer to the point sheet
- The results of the Career Center assignment
- Portfolio Assignment (below)
- Go to one of the two websites listed here and watch one of the live videos on the surgical procedures listed. Keep an eye on what the surgical technologist is doing. You may choose between the C-Section delivery at http://www.or-live.com/stmarysmadison/2089/ -or- the total knee replacement at http://www.or-live.com/shawneemission/2225/
- Once done simply tell me what you thought about the procedure, can you see yourself working in this field?
- Tell me why you think you have the ability and the talent to work in this challenging field?
- Portfolio's may be presented at anytime during the year
- The program is full time
- Students are accepted into the Fall semester (August) for 11 months.
- The portfolio will be stapled in the top left hand corner (no binders please)
- Portfolios that do not include the background check will not be evaluated
- Applicants are responsible for all the contents of the portfolio and must be submitted in its entirety. The portfolio’s become the property of the College of Southern Idaho.
- Students will be notified by mail if they have been accepted into the program.
- Once admitted there will be new student orientation. New student orientation is mandatory. If orientation is missed, you will lose your place in the program
A Grading sheet will be used to award points to the student’s portfolio (download grade sheet):
- The grading sheet will be used on each applicant to tally points
- Points awarded based only on the entirety of the portfolio
Applications are taken year round for placement in the fall semester full time program.
- Students are encouraged to apply at any time, however applicants must have minimum of 65 points to be considered for fall placement into the surgical technology program.
- If there are not 16 qualified students in June, applications will continue to be evaluated until the class is full.
- In the event that there are more qualified candidates than seats available, the remaining applicants will be placed on an alternate list for that year.
- As of January 2009 there is no longer a formal interview process as described in the 2009-2010 Catalog.
